How do I make an appointment?

What do I need to bring to my appointment?

Please be sure to have your current insurance card, a list of medications you are taking, a list of medical problems or questions you may have, and any forms you need filled out. Co-pay or co-insurance will be collected at the time of your visit.

How do I get my prescription filled?

We request that you contact your pharmacy for prescription refills. Please remember to allow up to 24 hours to process requests. We require patients taking prescription medications be evaluated once a year.

Why do you require annual visits to the physician for prescription refills?

We require that patients taking prescription medications be seen by their physician or nurse practitioner once a year for any prescription refills. We need to re-evaluate your health status. We may also determine if there is a new medication that would better suit your needs.

Do you bill my insurance?

We bill your insurance company based on the information you give to us. We bill the insurance within a few days of providing the service to you.

How much will I owe?

Your co-payment will be collected at the time of your office visit. You will be invoiced for any remaining balance after our billing department receives determination from your insurance company. You can call your insurance company for an explanation of your benefit program. The phone number to reach your insurance company member services can usually be found on the back of your insurance identification card.

Why do I need a referral?

Various insurance programs require referrals for visits to a specialist and for diagnostic tests. Please check with your insurance company to determine the need for a referral.

What if I need a referral?

Please call our office as soon as you discover a referral is needed. Please have information, such as the date of the appointment, the physician, the physician's contact information, and the reason for the appointment, ready.

How long does it take to get my lab results?

The amount of time it takes for lab results to be processed is dependent upon the type of test ordered. Once our office receives the results, the doctor must review them. Once the doctor has done so, a medical assistant or your physician will contact you. If you are not contacted within 5 days, please call the office. Please be aware that patients insured with United Health Care, Health America, Health Assurance or Advantra Insurance must have their laboratory specimens drawn at a LabCorp Patient Service Center.
