Scheduling an Appointment
To schedule an appointment, please call the office. Let the staff know what type of appointment you are requesting and we will coordinate a convenient time for you to see us. The Pittsburgh location can be reached by calling 412-281-2575 and the Crafton location can be reached by calling 412-922-2111. 

Appointment Cancellation
As a courtesy to other patients, please give at least 24 hours notice should you have to cancel your appointment. If 24 hours notice is not possible, please call us as soon as you know that a cancellation is necessary. If you anticipate being more than ten minutes late for your appointment, please call. Repeatedly missing scheduled appointments, without cancelling or rescheduling prior to the start of your appointment, may result in being discharged from the practice. 

Our physicians want to be available for your needs. Unfortunately, when a patient does not show for their scheduled appointment, another patient loses an opportunity to be seen. If you are coming to our office as a new patient, failure to show up for your first appointment without contacting us prior will result in us not being able to reschedule you.

Form Fee
There is a $10.00 prepayment fee for form completion. Upon receipt of payment we will be glad to fill out your forms. Please allow us sufficient time to process these forms. In fairness to all scheduled patients, we are not able to accommodate completing these forms while you wait. You may either mail in, drop off or fax to our office and we will contact you upon completion. Please fill out all information required of you, the patient, prior to submitting the form to us. If you have a request from your attorney, please have your attorney contact us directly.

Presciption Refill Policy
When you require a refill of your medication, please contact your pharmacy or supplier and have them submit the request to us electronically. We will have the request authorized by your physician and promptly return it for processing. With all prescriptions, please allow 72 hours for processing.

Medical Records
Due to regulation under the Health Information Privacy Act, all medical record copy requests must be accompanied by the appropriately signed authorization, whether you are requesting copies for yourself or for your records to be transferred to another party. Records requests are processed by Diversified Medical Records Services through a secure electronic process.

If you are new to the practice, you will need to request to have your records sent to CPMA. Our office can supply you with a form that can be submitted to your current physican to obtain your records. Please be sure to sign and complete the form in it's entirety to ensure that your request can be processed.

Return Calls
Should you need to call the office during patient hours, your message will be recorded and your physician or a covering physician will call you back. In certain cases, these matters may be addressed by the support staff. In order to expedite your request, please be sure to let us know the reason for your call. Urgent calls during regular business hours will be addressed immediately by the physican in the office. If you are in need of immediate medical attention, please call 911. For all other calls, a response from the physician usually will be made by the end of the business day.
